IMPORTANT: This form must be filled out 100% and submitted to A-1 Alarm before we can activate your Alarm.com GPS account

Please install your GPS unit after sending this form and we activate your account
. We will activate your account within 24 hours of receiving this information and you will be notified by email that your account is active.
Once your account is active, you will be able to customize your alerts for each unit you have registered. All units are accessible on one account once registered.

By default and with the purchase of your GPS, you paid for your activation and are subscribed to the Quarterly Basic Plan. All accounts are invoiced quarterly or annually in advance. There is a 5% discount for annual payments.Your 1st invoice will be adjusted to reflect the time of quarter you began and it may be for slightly more or less than a full quarter. Quarterly invoices are due 3/31, 6/30, 9/30 and 12/31 each year. Annual payments receive a 5% discount when paid by the due date. If you wish to enroll in Auto-Pay, please fill out the credit/debit card information below. Your credit/debit card will be charged on the due date of each invoice. Invoices are emailed 30 days in advance.
